Patient Activation Measure Intern

Sadler Health Center is a Federally Qualified Health Center (FQHC) that provides accessible primary care, behavioral health and dental services to insured, uninsured and underinsured individuals at two locations: Carlisle and Perry County. Sadler’s mission is to advance the health of our community by providing integrated, high-quality and compassionate care.
Our continued pursuit for excellence in patient care and patient engagement led to our partnership with the Medical Group Management Association (MGMA) and Insignia to introduce the Patient Activation Measure to Sadler Health Center’s patients.
PAM® is an evidence based predictive survey that assesses an individual’s engagement level and provides coaching interventions to increase activation and improve outcomes. The survey assesses the patients’ knowledge skills and confidence in managing their own health. PAM® identifies where an individual falls within four different levels of activation. Each levels is mapped to hundreds of consumer health characteristics for several health conditions. The tool is proven to predict future ER visits, hospital admissions and readmissions, medication adherence and more; which gives providers and health coaches’ insights to more effectively support each individual taking each patient from disengaged to activated.
Students participating in the PAM® internship will be tasked with a variety of responsibilities including but not limited to:
  1. Selecting and flagging patients in the Electronic Medical Record system based on eligibility criteria set forth for the PAM® survey.
  2. Administering the PAM® survey in person or over the phone.
  3. Inputting activation scores and levels in the patient’s Electronic Medical Record.
  4. Tabulating and analyzing survey data.
  5. Assisting in formulating and implementing organization wide continuous improvement initiatives related to Patient Activation Measure.
  6. Assisting in developing and tracking quality and practice management metrics related to the Patient Action Measure (e.g. hospitalization rates, ER admittance rates, no show rates, medication adherence…etc.)
  7. Periodic re-assessment of patients using the PAM® survey.
